It seems you cannot add the locations column if there are any menu items assigned to a location. If you delete these records from the locationables table the error disappears. Otherwise, there’s no way to fix this problem that I have discovered.
In short, you cannot assign a menu item to a location. This doesn’t appear to be a problem for categories where you can assign a category to a location.
